Collect Multiple Responses Into Single Column


i need form can collect multiple names , phone numbers , save data in 2 columns. 1 column names , 1 column phone numbers.

 

how can this?

you use formula in "view responses" table create single column concatenate other columns 1 - see response post earlier should answer inquiry well: http://forums.adobe.com/thread/1351642?tstart=0

Thread: Twinview issues

hi all, recovered white screen of death having reinstalling nvidia drivers via command line in recovery mode. when rebooted after that, fine, except 1 of monitors not displaying anything. ran sudo nvidia xconfig, created conf file me, issue not resolved. in x server settings, secondary monitor (the 1 not displaying), detected , enabled in twinview, resolution not detected , screen black. if try switching workspaces, little window showing workspaces show second monitor sitting next monitor, same in x server settings. options have resolving this? lot!
